I'm in the US so it may be different, but all the girls I know charge extra for anal on top of whatever their normal fee is.  And GFE/PSE typically charges the same rate (unless it's an actual porn star, then they charge absolutely insane rates haha).  I do GFE and not PSE just b/c, like you said, PSE is incredibly demanding and just too rough for me, but I charge the same rates as the girls who offer PSE.  Then anal is an extra $50, and most girls I know charge an extra $100 but I actually enjoy anal and love when a client wants it so charging a bit less is kind of an incentive :-) 

Do you feel like you have to offer PSE to remain competitive?  It sounds like you dread it and so many guys love the intimacy aspect of GFE.  Maybe you don't have to have a PSE option?  There are so many gray areas with these acronyms, too.  You could be GFE but still include certain PSE aspects that you're comfortable with as a nice surprise without advertising them and feeling like you have to do full PSE.
