I've seen ones like this quite a few times. They don't go anywhere near my mouth uncovered. What surprises me is that they seem to think it's normal? They need to see a doctor really and have a small procedure done to pull it back.

If the foreskin physically can't pull back it's a condition called Phimoses. It can be sorted by surgery. I first saw it years ago n was freaked out n had to find out more when I got back from the booking so did a bit of online research. I've seen maybe only one or two more cases of Phimoses since then in nearly 8yrs now. I really don't know why these guys don't get it sorted out then at least they can fuck and feel more normal about themselves. It can cause a lot of pain if they attempt to pull it back n doesn't allow for proper penetrative. Who would want to put up with that when it's so easily remedied? Honestly.
