I've let this through only because the punter knew about this beforehand and I think it's reasonable to expect him to mention it if having unprotected oral. However there is nothing to prove he was 'making up an excuse'; blood in semen isn't that rare and can indicate all sorts of things including recent prostate surgery and suchlike.

The way to avoid getting a mouthful of something you'd rather not is to not offer unprotected services - it's very easy to prevent. Bear in mind that the next punter with a similar problem might not know anything about it.
