A couple of weekends ago, I was at something that had a session on them, run by someone who's day job is promoting them. As such, she had to push the official way of using them.
It was very amusing that the two of us (I know them, but I've never been sexual with them) in the dozen or so in the audience who have used them a lot had the opposite advice: ignore the instructions! Specifically, chuck the inner ring away. You're then left with a choice: vaginal / anal pouch or a baggy condom. The latter was our preference. They're extremely easy to put on larger and / or 'interestingly shaped' penises, and provide more sensation to that person.
If you're going to use them, you definitely need to practice with someone you trust and who can say - if you're following the instructions - 'the inner ring has slipped, we need to stop' without them demanding refunds etc. Once you've used them successfully ten or so times, you can then try with a client. As said, you need to have your fingers around when the initial penetration happens to ensure that it's not going outside the condom.
Oh, for the past few years, they've been made of a different (still non-latex) material and even when used per the instructions are no longer noisy.
