A psychiatrist is a clinical doctor, they make diagnosis and prescribe medication for people with mental illnesses. A counsellor is someone that you can talk to about your life and stuff. From what you've said I feel it may be the latter you're looking for.
However if you think you have an illness that needs diagnosing and treating then a psychiatrist is for you. They then prescribe therapies that that would help. However what you discuss with them r e. Sex work will be on your medical records. A counsellor wouldn't work like that. Nothing you discuss goes on a medical record.
