I am the same MD, I ask them to couch and most do but many don't! I wear gloves and use condoms and am ultra paranoid about hepatitis b so make sure my resistance is good at the clinic x

I don't know anything about veins, etc.

But what I have done is to play around a bit with fingers (men's fingers, not my own.) Lots of lube and very gentle fingering to get used to the sensation. And I've used toys on myself. -slow deep breathing on your part
-take it slow stop/start and control the client
at all times with your hand.

Definitely go for a check, as if it's a haemarrhoid or something - ouch!

I'd start working your way up with increasingly large butt plugs, until you can comfortably insert one equivalent to a small-average cock. As others have said, use tons of lube, preferably desensitising to begin with, and you can also get relaxing sprays and creams.

I think if you are not comfortable with it in your private life, or experienced, then don't do it.

I used to offer anal all the time and loved it. I gradually built up with various sizes of butt plugs. You can get some great ones at lovehoney.
 
I've stopped offering anal so much now as I had some over zealous clients and I started getting swollen veins (hems). They've cleared up now but I don't want them to come back.
