I'm kind of glad I had the S.T.I. in the eye scare (not sure if it was, got the doctor's on monday).
Because of that, it made me think about the risks associated with reverse oral, and a client wanting to do that may have the beginnings of a cold sore that I can't detect.
Also, even if they were aware they had a cold sore developing, would they tell me? 
Call me cynical but...
It's made me realise there's no such thing as safe sex (ie. what you wrote about the use of dental dams) just assessing the risks and making it as safe as possible.
Until somebody invents a full body condom...
